Become a New Patient
If your thinking about having a nutritional consultation, you may make an office visit appointment with the nutritionist if you are in the
Baltimore area or if not you may make an appointment for a telephone consultation. The initial visit is about 1-1.5 hours long
whether in the home or over the telephone. To be prepared you will need to complete some paperwork prior to attending your first
appointment. Please print these forms from your computer then fax or mail them to our office. This will allow you more time to talk
to your nutritionist during the first visit rather than filling out forms.
Instructions to Become a New Patient
The following items will need to be sent to us prior to your appointment:
1. Food Journal Assessment
2. Blood Work Assessment
3. Functional Assessments
4. Metabolic Assessment
During your initial visit you will be able to discuss what your goals are and what concerns you have. The nutritionist will thoroughly
review your paperwork and discuss all of your assessments including clinical, body composition, past medical history, current
medications or supplements you are taking, lifestyle, food journal assessment, blood work assessment, functional and metabolic
assessments. A treatment plan will be provided to meet your goals and will consist of an integrative approach that includes
conventional, functional, complimentary and alternative evidence-based nutritional therapies. The nutritionist will provide written
educational materials including but not limited to: foods to add, limit or avoid, sample meal plan, a specific therapeutic meal
regimen, blood lab value descriptions, food-medication interaction or contraindications and supplement recommendations which
may include herbs, vitamins, minerals or any other nutritional aid if necessary. The nutritionist may recommend additional blood
tests or refer you to a specialist if needed. The nutritionist may also want to contact your doctor for discussion if needed. A follow-
up appointment may be recommended for review, support and re-evaluations so that you can meet your goals or continue to
improve your medical or health condition.

1st Visit Fee 125.00
Follow-Up Visit Fee 60.00
Payment Methods
For all phone consultations payment is made prior to your appointment. You may send a check or money order to our address.
For home visitations cash or check will be accepted in person.
Insurance Coverage
Currently, we do not except insurance. We will be happy assist you by providing the appropriate documentation that you may send
to your insurance company or flex accounts for reimbursement.
